The game is introduced with the following story to set the mood: After having spent several weeks traveling in Europe on business, you arrive in England for a bit of a break. You decide to take a week off. Well, you need it! However, you meet a very old friend in the local drinking hole who is in need of your help. He has just been invited to spend a week in a villa on the coast in southern Spain by a beautiful and very rich young Spanish girl, Conchita. The problem is, he has a milk delivery round and cannot find anyone to stand in for him. Yes, you've guessed it, he asks you if you would do the round for him. Now, if you had a choice, you would prefer a week in Spain, but Conchita seems totally besotted by the milkman. Just your luck. Anyway, things perk up a bit when he explains that the life of a milkman can be very varied! Now you're interested. Although the outrageously early morning start leaves you a little cold, after dropping your friend and his Spanish companion off at the airport at 4 am, you're ready to start the round. Apparently, the milkman has kept a record of his, err, regular haunts in some sort of code. Your task is to decipher his code and sample the delights they offer. You must set the day, choose a street, and then a house number. From then on, you are on your own!
To succeed and experience an exciting encounter with one of the customers, the player must deliver milk to the correct house on the correct day. However, things can go wrong—at one house, the player might be tricked by a thief and end up in the hospital. Upon successful completion, the player is rewarded with a special surprise, which aligns with the tone of the game.

This version of Jo Guest in the Milk Round was designed for personal computers with operating system MS-DOS (Microsoft Disk Operating System), which was operating system developed by Microsoft in 1981.
